HOW LIFELOCK WORKS
LifeLock monitors your identity.  When activity occurs involving your information, you’re alerted by email, text or a phone all. You can respond to confirm whether the activity is legitimate, and if it’s not, a U.S. based LifeLock Identity Restoration Specialist will help you resolve the issue.  There are three LifeLock options for you to choose from. Please refer to the more detailed summaries posted on our intranet.

LifeLock Benefit Elite protection is aimed squarely at what matters to employees: Protecting identities and protecting nest eggs. LifeLock Benefit Elite protection helps detect potential fraud and brings it to the attention of employees through alerts via email, text or phone.

The LifeLock Advantage Plan is an enhanced identity protection plan that offers the features most people want and at a price to fit your budget. It includes bank account protection and credit score and reports.

3. LifeLock Ultimate Plus
LifeLock Ultimate Plus plan provides peace of mind knowing you have the most comprehensive identity theft protection available. Enhanced services include bank account application and takeover alerts, online credit reports and credit scores. LifeLock Ultimate Plus also has a complete list of identity theft protection features that includes everything from lost wallet protection, to black market website surveillance and daily three-bureau credit report monitoring.

This plan will help pay you a percentage of the maximum coverage you choose. Diagnosed illnesses like heart attack, stroke, Alzheimer’s disease and cancer are among those covered. Rates will vary, as they are based on age, smoker status and family size.

These plans can be a great addition to your medical plan. As you may know, hospital visits can be costly, so it’s easy to see why having hospital indemnity insurance can make good financial sense. Critical Illness insurance will also compliment your medical and disability income coverage, which can ease the financial impact of certain critical illnesses.

Coverage is guaranteed provided the associate is actively at work and the dependent is not subject to a medical restriction as set forth on the enrollment form and in the Certificate.
Your Initial Benefit provides a lump-sum payment upon the first diagnosis of a Covered Condition. Your plan pays a Recurrence Benefit for the following Covered Conditions: Heart Attack, Stroke, Coronary Artery Bypass Graft, Full Benefit Cancer and Partial Benefit Cancer. A Recurrence Benefit is only available if an Initial Benefit has been paid for the Covered Condition. There is a Benefit Suspension Period between Recurrences. Initial Benefits and Recurrence Benefits will be paid until the Total Benefit Amount has been reached. The maximum amount that you can receive through your Critical Illness Insurance plan is called the Total Benefit and is 3 times the amount of your Initial Benefit. This means that you can receive multiple Initial Benefit and Recurrence Benefit payments until you reach the maximum of 300% or $30,000, $75,000 or $150,000.
